We collect the answers you choose to provide, including product interest, product priority, requested coverage-amount range, healthcare role, planning goals, existing benefits, age range, a broad gross monthly income range (with an option not to say), state, name, email address, phone number, preferred contact method and preferred contact time. We do not request an exact income amount. We also record consent selections and the versions of the consent, privacy policy and terms shown at submission.
Information this form does not request
The review form does not request a Social Security number, driver’s license number, medical records, diagnoses, prescription history, banking information or payment details. Please do not send that information through this form.
Technical and campaign information
The site may record a limited landing path, referring origin, campaign parameters, approximate request metadata and technical security signals. Campaign values are normalized before storage, and the form is not intended to store full referring URLs or free-form query strings that may contain personal information.
How information is used
Submitted information is used to review the inquiry, confirm whether an appropriately licensed producer is available in the visitor’s state, follow up using the selected method, maintain consent records, protect the service from abuse and improve site operations. The server derives a review path from the product interest selected so the inquiry can be routed for a product-specific licensed review. That review path is not a health score, underwriting decision, eligibility result or product recommendation. The income range is not used by this site to make an underwriting, eligibility, suitability or approval decision. A form submission is not an insurance application.
Storage and service providers
Form information is written to the site’s durable lead store. When a delivery integration is configured, a limited group of service providers may process information to host the site, protect it from abuse, deliver the inquiry or support follow-up. Those providers’ own terms and privacy practices may apply. This draft does not make an unverified promise about sale, sharing, retention or deletion practices; the owner and counsel must finalize those business-specific terms.
Calls and text messages
