This policy is a working draft prepared for review by the agency’s licensed principal and legal counsel. It must be finalized before the site accepts production traffic.
Information we collect
When you submit the review request form we collect the answers you provide: your healthcare role, planning goals, benefits you already have access to, age range, comfortable monthly budget range, state of residence, name, email address, phone number, preferred contact method and preferred contact time. We also record your consent selections and the versions of the consent, privacy and terms text shown to you.
Information we do not collect
This form never requests a Social Security number, driver’s license number, medical records, diagnoses, prescription history, banking or payment details. Please do not include that information in any free-text field or message.
Marketing attribution
We store the landing page, referring URL and any campaign parameters (utm_source, utm_medium, utm_campaign, utm_content, utm_term, gclid, fbclid) present when you first arrived, so we can understand which channels reach healthcare professionals.
How we use your information
We use it to review your request, confirm producer licensing in your state, contact you using the method you selected and discuss whether life-insurance options may be appropriate. Submitting the form is not an application for insurance.
Who we share it with
Your information is stored in our secure database and synchronized to our customer relationship management system so a licensed professional can follow up. We do not sell your personal information. Analytics tools, when enabled, receive only non-identifying interaction events — never your name, email address or phone number.
Text messages and calls
Automated-contact consent is optional and is never a condition of purchase. Message frequency varies and message and data rates may apply. Reply STOP to opt out of text messages at any time. You may revoke consent by contacting us.
Your choices
You may request access to, correction of or deletion of the information you submitted, and you may ask us to stop contacting you. Contact information is listed in the site footer once verified.
